Assessments

SASS provides initial comprehensive assessments to determine the needs of each individual client and develops an appropriate plan of care. SASS also provides ongoing case management for individuals who are determined to need the services based on the assessment.

 

 

 

 

Ongoing Case Management

Ongoing case management services are provided by licensed social workers and include services such as advocacy, coordination of services, and ongoing resources and referrals as needed. Other services include relocation assistance and medication management. SASS also provides assistance with setting up power-of-attorney health care documents. All clients have emergency access to a 24/7 on-call social worker.

 

 

 

 

Supportive Home Care

Our ongoing supportive home care services provide assistance with personal cares, shopping/errands, community outings, companionship, meal preparation, and skills training related to activities of daily living.

 

 

 

Other Services

SASS provides home studies for guardianships, home safety assessments, and comprehensive evaluations as requested for court proceedings

 

 

 

 

Therapy Dog

Visits from our therapy dog, Cooper, are available to ongoing clients upon request.
